The uniformly good prognosis of the JPA is often lost, when this lesion presents in less resectable locations like the hypothalamus.

Hemispheric Pilocytic Astrocytoma

When the JPA presents in the cerebral hemispheres, especially in older children and young adults, the diagnosis may not be obvious. This particular example has the classic "cyst with nodule" morphology. However, there is marked variability in shape, and in this location, there can be significant surrounding vasogenic edema. That may suggest an aggressive glioma, like a GBM.

Angiographically, the JPA is often an avascular or hypovascular mass.
